ICF/ID Team Leader 2nd Shift

Description:

OVERVIEW: The Team Leader provides guidance and leadership to ICF/ID staff with the goal of providing an organized and efficient program to individuals served. The Team Leader plays a key role in new hire orientation and the ongoing training needs of ICF/ID staff.

Duties:

DETAILED JOB DUTIES AND RESPONSIBILITIES

Be available for on-call duties as needed.

Participate in IPP meetings and provide input to the ICF/ID Director for the development of individual goals and objectives. Provide necessary reports.

Find a replacement if unable to work a scheduled shift. Follow the call-in procedure.

Follow individual program plans and implement programs as written. Provide guidance and leadership to SLS staff to ensure correct implementation of all programs, procedures, etc.

Complete Range of Motion exercises as written. Provide guidance and leadership to SLS staff to complete.

Engage individuals in purposeful, meaningful activities. Teach individuals rather than doing for them. Provide guidance and leadership to SLS staff in these areas.

Comply with all applicable policies, practices, and laws, and with the regulations provided by any governing bodies, such as Department of Human Services (DHS) and Department of Inspections and Appeals (DIA).


Give input into evaluations of SLSI and CMA’s.

Assist the ICF/ID Director and Shift Supervisor in training staff on programs by providing on-going training to SLS I staff.

Provide orientation to new staff within 10 orientation shifts. Ensure timely documentation of new hire training/orientation.

Oversee daily routine of ICF/ID program and ensure that active treatment schedules are being implemented consistently by staff.

Go on appointments as needed or assign appropriately.

Work weekends and holidays as scheduled.

Provide leadership and direction during fire and tornado drills and during emergency situations.

Perform direct care duties, including assistance with bathing, toileting, meals, repositioning, lifting, etc.

Review e-doc to ensure necessary documentation is complete.

Communicate with individuals served to evaluate needs and make recommendations.

Provide for the safety of the individuals served with the least amount of intrusion as possible.

Report issues of concern to ICF/ID Supervisory personnel. Offer ideas and suggestions for resolution of concerns. Use appropriate methods and channels of communication.

Provide guidance and leadership to provide individuals served with appropriate and meaningful leisure activities.

Be an active advocate for individuals receiving services to have greater control over their lives. Encourage individual choice and decision-making.

Apply rights restrictions only as determined necessary by the interdisciplinary team and included in the individual plan of persons served.

Accurately administer medications as ordered. Complete treatments and dressings as ordered.

Use judgment in monitoring and evaluating individuals for PRN medication use.

Complete required charting and data collection (e.g. MAR, side effects sheet, report book, PRN sheet).

Monitor individuals for side effects of medications.

Order medications and medical supplies as needed.

Keep med room clean, stocked and organized.

Notify physician of illness or injury as needed. Transcribe orders from pharmacy as indicated.

Assist in food preparation, following approved menus, physicians’ orders, and health and safety policies.

Ensure proper food storage and handling procedures are followed.

Ensure all dishes are cleaned, sterilized, and stored properly.

Other job duties as assigned (including: personal cares, bathing, personal hygiene, checking vital signs, and maintain a safe environment).

Qualifications:

QUALIFICATIONS:
• One year’s experience or education in working with individuals requiring 24-hour support.
• High School Diploma or GED.
• Certified Medication Aide (CMA) in Iowa or able to obtain certification.
• Supervisory and leadership skills/experience preferred.
• Basic computer skills including MS Word.
• Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
• Valid driver’s license and driver’s record deemed acceptable by DAC, Inc.
