| The Direct Support Professional (SCL) is responsible for assisting and supporting individuals in reaching their self-determined goals and outcomes and performing other duties as assigned. This is a 2nd/3rd shift, part time position in Anamosa, IA. |
Duties: |
Produce and maintain, as required by state regulations and company policies, records of individual care, condition, progress, or problems to report and discuss observations with supervisor or case manager.
Provide training that is needed and wanted through the implementation of the individual's Individual Service Plan (ISP).
Understands, follows, and advocates for the individual's rights.
Ensures individuals' health, safety, community living, and welfare needs are met.
Assist individuals in exploring and achieving hopes, dreams and goals.
Participate in the ISP process as necessary.
Provide incidental transportation as necessary.
Other duties as assigned by department or shift.

Qualifications: |
High School diploma or equivalent required.
Must be at least 18 years of age.
Ability to pass TB test, physical, and drug screen prior to hire and as required by regulation and/or policy.
Must be able to lift 50 pounds.
Ability to stoop, bend, twist, kneel, push, pull, sit on floor, and walk up and down steps on a regular basis.
Possess a valid drivers license and current auto insurance.
Satisfactory results on background checks with previous employers, criminal and abuse registries, etc.
Effective organizational and time management skills
Ability to complete required agency and department trainings within 90 days, and ability to maintain and utilize skills taught.
Ability to communicate verbally in person and by phone.
Ability to read, comprehend, and write simple instructions and notes.
Ability to comprehend and implement department, state and federal rules and regulations.
Ability to comprehend and implement each individuals programs and procedures.
Ability to act responsibly in a crisis, use good judgment skills, and willingly perform restraining techniques as needed.
Experience working with individuals with disabilities preferred.
Ability to learn about and train on daily living skills including cooking, household chores, etc.
Ability to work in the environment of the home assigned.
